Health Educator, Community and Worksite Health

POSITION TITLE: Health Educator, Community and Worksite Health (Full-Time/Education Department)

POSITION SUMMARY:

The purpose of this position is to provide cardiovascular health promotion/disease prevention activities and interventions for individuals and the community. The Health Educator is responsible for direct lifestyle/behavior change counseling via health fairs, health screenings and health and wellness programs. The core areas of focus include physical activity, healthy eating, tobacco cessation, weight management and stress management. This person will also assist the Manager of Health and Wellness Programs with grant writing and the development, promotion, implementation and evaluation of the department’s programs, products and services.  Responsibilities include the maintenance of the program database of activities and participants. All functions and behaviors are applicable to community, worksite and clinical settings.

PRIMARY RESPONSIBILITIES:

  1. Program planning, implementation and evaluation
    1. Implement new and existing programs and service designed to promote health through education, behavior change strategies, policies and environmental change.
    2. Provide direct education/lifestyle behavior change counseling.
    3. Assist with generating outcome-based measurement tools.
    4. Assist with employee health and wellness initiatives.
    5. Utilize social media and web-based learning approaches, as needed.
    6. Initiate and maintain relationships with community partners.
  1. Health communications
    1. Assist with the development and/or revision of heart health education materials and publications for use with various programs and services.
    2. Assist with program marketing and promotion.
    3. Assist with website content, including video content.
  1. Grant support
    1. Assist with grant funding through public health, private foundations, corporate giving entities and other sources to support new and existing program initiatives through the Foundation and/or in partnership with other organizations.
    2. Assist with grant implementation.
    3. Assist with grant evaluation and reporting.

DESIRED EXPERIENCE/SKILLS/CHARACTERISTICS:

Desired Education and Experience

  • Bachelor’s degree in health education, exercise physiology, health promotion, or other health-related field.
  • 2 years experience in health promotion or preventive counseling experience on health or wellness-related issues.
  • Project management experience.

Knowledge, Skills and Abilities

  • Knowledge of guidelines and standards of care for prevention and treatment of cardiovascular disease.
  • Ability to manage multiple projects, take initiative, solve problems, improve processes, work independently and on a team.
  • Strong customer/client service orientation.
  • Excellent presentation and facilitation skills, strong organization and communication skills.
  • Strong computer skills, familiar with Microsoft Office products, social media and web-based learning.
  • Ability to deal with ambiguity and changing priorities.
  • Ability and willingness to work occasional nights and weekends.
