In 2003, the Minneapolis Heart Institute
Foundation established the ReachOut for Heart Health Mini-
Grant Program to provide support, through funding and/or
direct services, to recognized at-risk communities involved
in local cardiovascular health promotion and disease prevention
projects or events.
Applicants have the opportunity to define
the extent of support required through cash as well as direct
services available through the Foundation’s HeartMatters program. Individual
grant awards will range from $500 to $2500 each. Funding/support
for 2005 distribution has been established at $25,000 thanks
to the generous support of sponsors.
Any community-based organization currently
involved in improving the health of populations of color,
American Indians and/or rural communities is encouraged to
apply. Projects must focus on recognized risk factors associated
with the development of heart disease and/or related conditions
(i.e., diabetes, high blood pressure, physical inactivity,
obesity, high cholesterol, etc.). Both new and established
programs will be considered for funding. All Letters of Application
must detail and demonstrate cooperative and collaborative
relationships among diverse organizations within and outside
the target community. Single agency proposals will not be
considered.
